This tool is a sample of a program evaluation survey that can be used to evaluate a wide range of issues about the effectiveness of an employee wellness program. The survey answers all the questions that senior managers usually want to know about their wellness program. Qualitative and quantitative indicators can then be derived from the answers provided in the survey instrument. Each section of the Annual Program Evaluation Survey (APES) helps answer the major evaluation questions most managers want to know about their wellness program’s performance.
